Pregnant women who want to take a more active role in their pregnancy while receiving ongoing support from their LWH physician and clinical team are good candidates for the Ochsner Connected MOM Program.  At your first obstetrics visit at Louisiana Women’s Healthcare, you and your physician can discuss your specific prenatal needs and decide if Connected MOM is a good fit for your pregnancy. The Connected MOM program at Louisiana Women’s Healthcare allows pregnant women to monitor their readings alongside their physician and clinical team through the use of advanced digital prenatal tools.  If enrolled in our program by your LWH physician, a digital scale and blood pressure cuff will be mailed directly to your home. You will use these tools to submit regular readings to your LWH physician and clinical team. The tools are synched with your smartphone and will automatically upload to your electronic health record for your physician to review.  As your pregnancy progresses, your LWH physician will use the health information gathered from these tools to proactively address any potential issues and tailor the best plan of care for you and your baby.

Once your physician has enrolled you in Ochsner Connected MOM, you will need to log-in to your MyOchsner account to confirm your participation in the program. After signing a consent form, you will be asked indicate the mailing address of the location where you would like to receive your tools.  You can expect your digital tools to arrive within 7-10 days. To officially complete enrollment, you will need to use these tools to submit your first weight and blood pressure readings to your LWH care team. The Connected MOM program is intended to enhance the level of care and access to your LWH physician and clinical team throughout your pregnancy.  Once enrolled in the program, check with your LWH physican to see if you’re an appropriate candidate to consolidate select appointments.  You may also speak with your LWH physician about whether MyOchsner Video Visits are right for you.

Monitoring an expectant mother’s weight and blood pressure are two key components for a healthy pregnancy.  Abnormal levels can lead to adverse maternal or fetal outcomes such as preterm delivery, pre-eclampsia, and low birth weight.  Through the Connected MOM program, these issues can be identified earlier, not just when a woman shows up for her routine prenatal appointment.  Proactively addressing these issues can improve outcomes for mom and baby before, during, and after delivery.  

If your weight and or blood pressure readings are abnormal, you will receive a text alert with instructions to contact your physician at 225-201-2000. After hours, holiday and/or weekends, you should call our after hours line 225-922-3766 for emergent needs. If your physician determines a need to change your care plan, necessary next steps will be communicated to you by phone or through the MyOchsner patient portal.  It is very important that you enable MyOchsner notifications to receive important alerts from your Connected MOM care team.

While participating in the Connected Mom program, you will be able to send secure messages directly to your care team anytime via the MyOchsner patient portal.  You may also reach your LWH care team by calling (225) 201-2000.

Connected MOM has a dedicated technical support team available to you Monday through Friday, 8am-6pm. If anything goes wrong with your equipment or you need help remembering how to use the technology, you can call 1-866-517-1377, and we will be available to troubleshoot with you.  If you have general questions about your Connected MOM digital tools, our C.A.R.E. The Connected MOM tool kit is yours to keep upon completion of the program. While the equipment will no longer automatically send readings to your electronic medical record, you are encouraged to continue using the equipment to stay on track with your health goals.
